The CLOZAPINE Prescription Access System (CPAS) is the patient registry for patients treated with The Mylan Brand of Clozapine. CPAS was designed to simplify required patient monitoring and expedite required reporting and patient history reviews. CPAS may be used to monitor patients who are new to clozapine therapy and patients who are switched to The Mylan Brand of Clozapine from another manufacturer’s clozapine. In either case, CPAS will work in cooperation with the National Non-Rechallenge Master File.
CPAS generates an efficient, methodical display of patient, pharmacy, and physician data that is available in real time 24 hours a day. It provides White Blood Cell (WBC) counts and Absolute Neutrophil Count (ANC) values every week, every 2 weeks or every 4 weeks. It also provides dosing and dispensing information, registration and monitoring forms, and preprinted WBC Count and ANC Reporting Forms for registered patients. It also offers a toll-free phone number24 hours a day, 7 days a week for registration and questions, plus a toll-free fax number for facsimile transmission of forms, if needed.

Do not use clozapine in patients with hypersensitivity to clozapine, myeloproliferative disorders, uncontrolled epilepsy, paralytic ileus, or a history of clozapine induced agranulocytosis, or severe granulocytopenia. Do not use simultaneously with other agents that can cause agranulocytosis or suppress bone marrow function.
