HOW TO REGISTER

New Patients and Healthcare Providers

All healthcare providers and patients must be associated with a closed distribution system, which means that physicians and pharmacists of patients new to clozapine therapy must be registered prior to the first dispensing of the clozapine tablets.  This is accomplished by completing a Registration Form.  The treatment eligibility of all new patients will be confirmed with the National Non-Rechallenge Master File.  The newly registered healthcare providers will be issued a registration notification by facsimile or mail.  Also, the physician will receive Patient-Specific WBC Count and ANC Reporting Forms for all newly registered patients.  To receive a Registration Packet please contact a CLOZAPINE Prescription Access System (CPAS) representative at 1-800-843-9915 or fax us at 1-800-843-9916.

Continuing Therapy Patients

Patients who are “continuing” clozapine therapy with another manufacturer may register differently.  A CPAS registered pharmacy may substitute The Mylan Brand of Clozapine tablets for another manufacturer’s clozapine tablets.  After dispensing the first supply of The Mylan Brand of Clozapine, the registered pharmacy simply forwards the WBC Count and ANC Reporting Form to CPAS where registration of the physician and patient will be completed prior to the next week’s supply of medication.  All of the registration information may be completed over the telephone for convenience.  CPAS does not require actual signatures of these physicians.  Since the only change in treatment is the manufacturer, CPAS maintains that initial registration with any registry signifies an ongoing commitment to adhere to the guidelines stated within the Clozapine Prescribing Information.  Once CPAS has obtained all necessary physician and pharmacist information, CPAS will send both the physician and pharmacist Patient-Specific WBC Count and ANC Reporting Forms.
