COURTESY REMINDERS AND ALERTS

Courtesy Reminders are provided as an additional precaution for monitoring patients.  They contain information about any procedures, actions, or Prescribing Information requirements that may be appropriate.

Alerts are provided when it is determined by the Clozapine Prescription Access System (CPAS) that a safety issue related to the program and defined by the Clozapine Prescribing Information may have occurred.

It is not the intention of CPAS to define, determine, or influence a physician’s or pharmacist’s care of the patient.  The intent is only to provide additional patient information for participating healthcare providers.  Courtesy Reminders and Alerts will be prompted by:

TELEPHONE REMOTE ACCESS DATA ENTRY

Automated telephone remote access allows registered healthcare providers to review the previous WBC count and ANC value of registered patients and to confirm patient reclearance codes.  This secure method of communication will also allow healthcare providers to enter all of the information contained on the WBC Count and ANC Reporting Forms C, D, and E.

SECURE ON-LINE ACCESS

Healthcare providers registered with CPAS can receive a username and password to access or report WBC count and ANC information for their patients treated with The Mylan Brand of Clozapine.   Access to this secure area of the website will allow healthcare providers to report every week, every 2 weeks or every 4 weeks WBC counts and ANC values, review patient files including the entire history of WBC counts and ANCs, dosage information, and confirm patient reclearance codes.
